Approved Driver Information
 
How to Become a District Approved Driver
 
1.  Complete the DMV Driver Record Request
2.  Complete the DMV Release Form
3.  Complete the San Lorenzo Unified School District Volunteer Driver Application
4.  Register on Family ID Bohannon Athletics District Approved Drivers
    • Upload a copy of your driver's license
    • Upload a copy of your insurance card
    • Upload your DMV Driver Record
    • Upload your DMV Release Form
    • Upload Volunteer Driver Application
5.  Hit Submit - Once you are approved you will receive an email from the Athletic Director
